That's where mine is, did you start with fresh source or a make clean?

I just did a complete download and install with only the changes 
specified on a stock RH 6 box with only the appropriate updates installed.

/usr/include/db1/ndbm.h
/usr/i386-glibc20-linux/include/ndbm.h

That's it for ndbm.h.
