The *very* best reference is "Building Internet Firewalls" by
Brent Chapman and Elizabeth Zwicky, published by O'Reilly (you
can order from their website http://www.ora.com/ or it is probably
at your local tech bookstore).

It has tables of the port usage for many common protocols, and enough
info to figure out how to deal with less common protocols. It also
has very clear descriptions of various firewall topologies. I can't
recommend this book enough!
