I already gave the link, but now I've looked at the rest of the site
(shame on me), off the main page is a "common product faults" and
non-random IVs are 6th on the list of both itnerop (RFC interpretation)
and  missing criteria requirements.  That page hasn't been updated since
last July though, I'll have to see if it can be filled with more recent
data.
