Our little IMGate project, "read my sig", uses postfix on FreeBSD as
relay-hub.
At our ISP, we have an client downstream from us on a leased line and
running Exchange on a public ip on DMZ. so we got none of your your NAT
pbs, but I cant see why a decent PAT box wouldnt let you put your
exchange on a private ip.
Our IMGate, sort of as a bastion SMTP host, relays all Exchanges in/out
mail, while doing anti-abuse defense on the inbound. Nothing special to do
in Exchange, other that configure the IMC to forward all outbound to IMGate.
