On windows machines I've found Process Explorer to be very useful:

http://www.microsoft.com/technet/sysinternals/ProcessesAndThreads/ProcessExplorer.mspx

It's basically Task Manager on steroids.  You can get a lot of process
specific information, including per-process CPU and memory usage
graphs.

Don't really know of any good automated tools though.
