Are you deploying on unix? I remember there was a case issue with that sample download (a while back)...Some places there's reference to fileio.swf and other places fileIO.swf.

The Apache uses Proxy-Pass and Proxy-Reverse for tunneling the requests from the machine in front of the FW to the protected Tomcat Server. But the SWF is on the same physical machine as the MXML/AS stuff. It is embedded by <mx:Loader> alomst identical to the description in the MM document.
