Did you get a chance to run this using JAWS? I think you will find some quick fixes you can make like when windows with many form controls come up, you can alert the screen reader user to enter forms mode manually. I would suggest using SWFObject to add your swf to the page. Also, you may want to look into the tab order for a couple of radio buttons you have.
