After resetting things for the umpteenth time, I switched back to output on VAC-2 and input on VAC-1 (from PowerSDR's point of view). That got it working. I have tried this combination before so don't know why it worked this time.
Now that the RX audio is working it is time to get the CAT stuff working. After that I can think about transmitting. I want to thank everyone who has replied with suggestions.
