According to the manual, this is likely caused either by high SWR or overdriving the amp:
*If the reflected power exceeds 50 watts the power level should be reduced until the reflected power indicated on the internal meter is 50 watts or less on peaks. At approximately 75 watts of reflected power the internal safety circuitry will disable the amplifier.* SO, 1st, check your SWR. Roll back "RF Power" - drive - and then gradually increase it until you're seeing the rated output from the amp. Then stop. Details on page 15 of the manual.
