I just commited some tweaks to magneto-switch.xml The way it works is the left mouse button selects the switch position up to and including the "both" position. The middle mouse button engages the starter. If you hold down the button, the switch returns to the "both" position but it will cycle between that and start. Alternately you can click once then click again to disengage the starter.
