I don't think there's a clear policy on this - either way works, as long as the patch files are clearly indicated in the STR... For myself, I tend to do one patch per problem, regardless of the number of files affected. If the patches are meant to address multiple problems, then I (personally) would go for one patch each - but then you can get into the situation where you have to apply the patches in the "right order" if they "overlap", so...
