I found the bug. As usual there is nothing wrong with fltk and it was a stupid 
oversight on the programmer's side.

PS:
the problem:
by adding a menuitem one of the calls to menu()[n] was overwriting a "0" item, 
causing some unexpected behavior.
the solution:
Banging my head against the wall.
