Sorry, this is my mistake. I had run the plaincode as well but just opened the 
file in a browser, which seems to have taken care of the missing head and body. 
Now that I've opened the file in an editor I see that it is stripped down.

I've attached a couple of screenshots to illustrate the issue. In the default 
view the top line isn't indented at all so the rest of the code appears to be 
indented too far. In the plaincode view the top line is indented too far and 
lined up with the content of the function.
