The typical thing to do w/ a TrueType font w/ arbitrary encoding is to use ttf2tfm to create a .tfm, work up a map file, .vf if necessary and to set up pdftex w/ where all of the above is located.

fontinst is needful when one needs to set up a traditional text oriented font, and can be useful for other tasks if one needs to set up complexities in a virtual font.
