Original SVG graphics, XML, and XSL (DocBook customization layer, you will still need the DocBook distribution XSL) used to produce the PDF document is downloadable from: http://www.getnet.net/~swhitlat/DocBook/XSL_DocBook_XML_example.tar.gz The error is reproducable every time.
