I could also reproduce the bug using FOP 0.20.5 and the provided bug.fo. It
occured on page 2 out of 8.

Same here for me (also using the command line option -d)

Haven't tried it.

Same here: the output in the Window of the AWT-Renderer looks fine. Only at
printing comes a problem.

If you (FOP Team) suspect that the problem lies in the AWT-Renderer, I could try
to investigate.
