api update patch
I've updated the documentation as discussed on fop-dev. I've removed
(http://xmlgraphics.apache.org/fop/dev/api-doc.html) since it's a little
misleading and I've added some information about the use of document handlers.
If you have suggestions of any amendments, please feel free to discuss them.
