NOTE: After at first installing Adobe's MakeAccessible, I couldn't figure out how to use it. In fact, after searching the menus, I couldn't even find it.
NOTE2: Be advised, that after running this plugin, I found a few Euro characters in my file that weren't there before. I had to scroll down the page, and then back up to see them. They showed up at the end of blocks, and I'm not sure why they showed up yet. We're a US company and don't make use of the Euro in our documents--for that matter we're not using the $ either...

Have you tried using the Adobe Make Accessible plugin? I realize you'll want to automate the process, and perhaps it's accessible from the command line? I'd be interested in finding out more about this if you find anything.
