Specify the heigth/width of your SVG with real length units:
<svg:svg height="22.5mm" width="45mm" ...
instead of pixels:
<svg:svg height="100" width="200" ...
You should use the same measurements explicitely on the
fo:external-graphics element referring to the SVG.
