There is a free word processor AbiWord (www.abisource.com) which appears to have a 'save as XSL-FO' plugin. I tried it but the output was a bit broken - missing close tags, etc.

The only free tool I know of is FOA (Formatting Objects Author). There are a couple of commercial products, but none of them are very good.
