You can try to place everything in absolutely placed block containers. However, because FOP 0.20.5 doesn't implement overflow clipping, nor enforcing max-heigth settings, you are probably out of luck with pure FO coding. You can try to use SVG and do the clipping in SVG, however, you'll probably have to rely on Batik's flowText extension and also be ready to wrangle with font and spacing issues. Recreating paper forms using FO isn't all that easy (and BTW I think it is also a really stupid idea).
