I am very certain that this is still a classpath issue. Looking into the JAI jars com/sun/media/jai/codec/FileCacheSeekableStream is in jai_codec.jar. If you do a jar tf on jai_codec.jar it is actually within the first 10 files listed.
I don't know why your java environment doesn't find it. May be you can describe how you invoke fop?
