Seems that when I create a link to an internal resource like below. This type
of link is the vast majority of links within out document; part names and part
number indexes, all look-aheads (I think that is the right terminology).

<fo:block font-family="Arial" font-size="8pt" font-weight="normal"
text-align-last="justify">
  <fo:basic-link internal-destination="SR1398-part-d64e2095-1046"
destination-placement-offset="0.00in">ABS Ring and Shaft Seal Assembly
    <fo:leader leader-pattern="dots" leader-alignment="reference-area"/>
     2-
    <fo:page-number-citation ref-id="SR1398-part-d64e2095-1046"/>
  </fo:basic-link>
</fo:block>

What happens is that the page-number-citation is not part of the link. Examples
below. The '|' in the text is to show where the link ends. The text is exactly
how it appears in the PDF.

Numeric index

009594 ............................................|. 2-2
009594 ........................................... 2|-10

Alpha index

Rear Brake Cam & Slack Adjuster, Streetside .........2|-31
Center Link Installation .............................................|. 1-7
Rear Shock Absorber ...............................................|. 2-3

I'll try and post what the area tree file contains for at least a couple of the
above, if that will help.
