Hi Peter To add to what Glenn already answered, we can tell that you are using Docbook XSLT to handle the XML to XSL-FO conversion.
Now, Docbook is one way to achieve that. Another would be to write your own XSLT stylesheets. Not every subscriber to this list is also fluent in Docbook, though, so in order to get help specifically on tweaking Docbook to change the output, it is recommended to post the questions on their mailing lists. Do mind that they also have an FAQ section, that they will expect you to have browsed before posting your questions.
