FOP doesn't support extended FO features for Indexing, see https://xmlgraphics.apache.org/fop/compliance.html For DocBook outputs there is a workaround http://www.sagehill.net/docbookxsl/IndexCleanup.html, which could be most likely adapted for your specific use case.
