Regular expressions are bound to type 3 grammar (regular gramar) which makes it difficult to cover this topic, where the number of opening and closed tags and comments needs to be considered.
Maybe we could simplify it and don't do switch syntax highlighting for <?php or <?= tags which are inside of comments. I've attached a simple example of my idea ( https://regex101.com/r/iF9wY6/1), but probably it's not worth the work to change that in pspad.
