Ah, I don't mind which rests; I'm not very good at SQL to understand the nuances of the alternative implementation; if both work, take the implementation you prefer. Nevertheless, I think the best approach would be setting the download name at the time of writing the link Download, to set the 'name' according to the context; that should be able to handle the case of multiple attachments (or files) having the same contents, but under different names in different contexts (wiki attachment, ticket attachment, repository files, ...
