We got around this by using different paragraph formats for Chap and
Appx headings, and then set up the TOC entries on the reference page
as follows:
Chapter <$chapnum>: <$paratext>\t<$pagenum>
Appendix <$chapnum>: <$paratext>\t<$pagenum>
