dmesg output here? Looks like there's no interrupts for the card. One other thing to try is to wait until after you've booted to insert the card (and after if_rl.ko is loaded). There's currently a minor problem with doing a kldload if_****.ko for a cardbus card that's inserted into the unit and has failed to activate at least once.
