Hi, I'm not sure how to help here; it looks like there's either an issue with the firmware or a physical issue with the cage. Though, another option you could try would be to download the latest driver from the Intel Download Center and see if that works. If not, then also try the NVMupdate utility from Download Center as well since maybe a firmware update could help fix the issue.
