# cd /usr/ports/sysutils/portupgrade
# make install clean
# rehash
# portupgrade mod_php4

The man pages for portupgrade will tell you handy little hints like if
you use:
# portupgrade -rR mod_php4
it will actually upgrade all of the dependencies for PHP (and anything
that depended on it too.)
