for months/years without doing so.

So I'm curious, how often do you keep your ports update, and what are
the reasons for doing so?

I update my desktops daily. The process is streamlined and doesn't need an eye on it. I'm updating servers weekly or so.

This means I have smaller blob of updates and almost never face large updates that broke everything.
