veja esse esquema de formatar a fita.. setar a densidade.. etc caso continue com o problema.. é possivel que seu drive esteja com problema mesmo. os leds dele não estão indicando limpeza (mesmo que não esteja, vc já tentou utilizar a fita de limpeza?)
