I run FreeDOS natively - no emulation, no VM.
To your second question - yes, INT13h functionality for USB is provided by
BIOS (or by CSM) so I do not use any DOS USB drivers.

Firstly, you say the laptop is fairly new, so I assume it is UEFI and your
"BIOS" (the thing that is providing the INT 13h functionality) is actually a
CSM? IOW, you are not running inside some sort of VM?
Similar question for the USB drives that you say work OK. The INT 13h
functionality there is also provided by the CSM and you're not using any DOS
USB drivers?
I will say at this point that the fact that the size is related to being 9 x
512 is probably not just a coincidence.
