yes the create_proc_info_entry function was added in 2.3.25
and the vm_offset was changed to vm_pgoff in 2.3.25 according
to linux cross reference...
So, I think you are correct...That must have been oversight.
I'm sure everyone knows about this site, but there is a wonderful
site called "Linux Cross Reference" that I have found is a very
valuable tool for these sorts of things.
