Nothing has changed in the configurable failover code for nearly a year and a half. The examples in the documentation SHOULD work. My suggestion is to try using configurable fail-over with ANOTHER module (e.g. the 'always' module), for testing. Once you've got it failing over, then switch to using the SQL module. If that stops working, then there's a problem with SQL, not with configurable fail-over.
