Here are some that could be the cause:
- the NAS is sending retry packets too fast
- the radius server takes too long to process the packets and send an Ack
back the to NAS
- the network between the NAS and the radius server can't handle the load
- bug in the NAS

I would say it doesn't really matter, as long as you have at least one of
them, you have all necessary session data.
