  See the WARNING message in the debug log you posted.  It should be clear.

  And why 2.0.4?

  I'll also note that you did *not* configure FreeRADIUS to use SQL.
Search for "sql" in raddb/sites-enabled, and un-comment the relevant
lines.  See also raddb/sql.conf
