i've got a similar problem in the first attempt to create this scenario. The problem was related to a field in the LDAP ldif that the switch doesn't accept (radiusenterprise policy). Now all the sequence seems ok, but Radius seems to send the authentication to switch (that reports port is authenticated) but not the VLAN ID :(
Do you have (or anyone) and example of PAP authentication that works? I think the problem is related to a misconfigured freeradius..
