According to Maxwell on IRC, --no-osb fixes it for him. I tried isolating
conditions which could lead to the problem, and here's what I think might
get us closer to it:

Maxwell is using Windows Server 2008 R2 SP1 without themes (the default).
I'm always enabling themes in my test virtual machines, which actually
affects the usage of the offscreen bitmap. IIRC, the server does make
extensive use of the offscreen bitmap cache for certain UI elements like
window buttons such as 'x' when themes are disabled. When themes are
enabled most of it is simply sent as bitmap updates.

For those who experience the issue, who has themes enabled and who doesn't?
Does using --no-osb work around the problem? I still wasn't able to
reproduce it
