Is there a possibility to activate all DTMF detection modes (in-band, SIP INFO & RFC 2388) in the same dialplan or maybe in the same extension of the dialplan?

The start_dtmf app looks for in-band DTMFs, so if you have to have that in order to hear digits from the other end then the other end most definitely isn't doing RFC2833. For the sake of testing, try sending in-band and see how the other end reacts.
