This can happy only in case a developer uploads a new binary of a package without incrementing the package release number, ie 1.0-1 again after 1.0-1, instead of 1.0-2. It is not something meant to happen anytime, but sometimes it happens in -current. Sorry for that, sometime we do mistakes. (And it was a user request to make pacman -Syy not the default, since it's way slower than -Sy.)
