I think a sufficient cause is that you are using a computer,
which depersonalizes the process of action degradoing into
rote repetition, and thus makes process mistakes even easier than
when one does something over and over again manually.
Beyond that generality, it's too easy to simply hit the
enter key when sending email, taking for granted that the addressee(s)
are the ones one intends and not the ones your email program's
automatic name completion feature selected.
