SmartDefense is not capable of blocking MSN messenger completely. If you don't grant MSNMS access, MSN messenger will try http access to MSN hosts and that way the user will be able to successfully use MSN messenger.
You should create a rule that blocks all your clients from contacting MSN messenger hosts via http or MSNMS.
