Matt, I have encountered this before too.
Turned out to be nothing with interfaces, NAT'ing or IP but with the default installed version of SmartDefense. ... Go figure :-S Either perform a SmartDefense update or turn off "General HTTP Worm Catcher". That did the trick with us.
