Do you have any rules that are disabled? If so remove them. Sounds crazy but I have seen this twice now in R65.

What I've noticed is that the mdq process is not running. There's also no fwssd process running. I've seen instructions to run "fw mdq" to restart, but that doesn't do anything (it gives me a help page on how to run the 'fw' command.) I'm not using a content checking filter. I'm just using the security server to take mail from our DMZ mail server, and route port 25 on the firewall to our inside mail server. What am I doing wrong?
