Depends on your exact requirements and where you want the block to take affect.
There are many ways to solve this issue but one common approach is to use a mail intermediary such as Baltimore's Mailsweeper in the DMZ which will act as middleman between your internal Mail system (Lotus Notes, MS Exchange or whatever). These intermediaries can also be used for virus checking etc. before the email hits your internal mail system. You can configure the system to forward blocked email to an administrator who can judge whether the applied rule was too harsh or not (delete email or forward to intended recipient).
