I had the same experience with eSafe version 3 in CVP mode. I went back to
version 2.1 and the problem went away. Version 3 running as a relay looks
like an elegant solution. The firewall is not involved in the virus
scanning. I plan to try this at some time in the future.

I would consider running esafe mail instead. It doesn't use CVP, its
basically a mail relay. Although it SHOULD be simple, CVP implementation is
not for the faint of heart!

I think your problem is with eSafe. I can't get v3.x to work reliably for
more than a few hours. I had to downgrade back to version 2.x in order to
get the mail flowing. As soon as I started the older service, the mail came
through.
