The Macally (1.1) USB card is flat and has a 2-port dongle. It fits 
under any of my other PC cards. Although it's not being manufactured 
any longer, you ought to be able to come across a used one for $30 or 
so.

I'd try the LEM swap list first, unless you are already an ebay user.
