The card is the Belkin F5D7010, which used to have a Broadcom chipset & operate with the Airport drivers (3+) but now apparently incorporates one from Ralink Technologies, http://ralinktech.com - click "support" at the top of that page and then "PCI/MiniPCI/CardBus" under the Macintosh heading to download the driver.

It says it's for 10.x on a G4 Titanium - works for me with 10.2.8 & Wallstreet.
Nice flat form factor.
Though if I'd known I was going to use a 3rd-party driver (instead of Airport) I would have looked into a card that has drivers under both OS 9 & X.
